For Receiving Facilities
Overview
The Baker Act Reporting Center at the de la Parte Florida Mental Health Institute receives forms/data on behalf of the Florida Department of Children and Families (DCF). The Baker Act data consist of data entered from these forms. These Baker Act data are used to create a statutorily required annual report [F.S. 394.463(2)(a)3(e)], as well as additional reporting to address a variety of policy issues. All Baker Act receiving facilities (publicly & privately funded) are required to submit Baker Act forms/data to the Baker Act Reporting Center.
The side tabs (to the left on this web page) are intended for Baker Act receiving facilities as needed. However, all questions and suggestions related to involuntary examination form/data submission should be addressed to the Baker Act Reporting Center.
Additional questions may be addressed to the Baker Act Reporting Center at bakeract@usf.edu or 813-974-1010. This email/phone number is monitored by Baker Act Reporting Center staff. Depending on the issue addressed, you will receive a response from Annette Christy (Center Director), or Sara Rhode.
